Our mission is to make hearing care accessible to everyone. We understand that the cost of hearing aids can be a barrier for many individuals and families. Through our assistance program, we provide free or reduced-cost hearing aids to those who qualify. Apply below to see how we can help you or your loved one hear better and stay connected.

Our criteria
Our program is designed to help those in financial need. To qualify, applicants typically need to:
- Demonstrate a financial need
- Provide proof of income and other required documentation
- Be a resident within our service area

How to Apply
4 Simple Steps
1. Download and Complete the Application Form
Click the “Download” button below to access the application. Fill out all required sections, including personal information, medical history, income verification, and your hearing aid preferences.
2. Gather Required Documents
Make sure to include:
- A copy of your W-2 form (or a recent bank statement if you do not have a W-2) for income verification.
- Medical clearance for hearing aid use, signed by your doctor (or complete the waiver section if you choose not to provide medical clearance).
3. Submit Your Application and Processing Fee
Mail your completed application along with a processing fee of $150 for one hearing aid or $300 for two hearing aids. The fee should be a cashier’s check or money order payable to Live Better Foundation (no personal checks accepted). Send everything to:

4. Wait for Our Response
After submitting your application, a Foundation representative will contact you to confirm receipt and provide additional steps if necessary. Please allow up to two weeks for processing.
