Our mission is to make hearing care accessible to everyone. We understand that the cost of hearing aids can be a barrier for many individuals and families. Through our financial assistance program, we provide free or reduced-cost hearing aids to those who qualify. Our program is designed to provide financial assistance to those in need. To qualify, applicants typically need to fill out an application form and meet the following criteria: 

- Demonstrate a financial need 

- Provide proof of income and other required documentation 

- Be a resident within our service area 

If you meet these criteria, we encourage you to apply! Each application is reviewed with care and compassion to ensure that we can help as many people as possible, including those in need of hearing aids.

1. Download and Complete the Application Form

Click the “Download” button below to access the application form. Fill out all required sections, including personal information, medical history, income verification, and your hearing aids preferences.

2. Gather Required Documents

Make sure to include:

- A copy of your W-2 form (or a recent bank statement if you do not have a W-2) for income verification.

- Medical clearance for hearing aid use, signed by your doctor (or complete the waiver section if you choose not to provide medical clearance).

3. Submit Your Application and Processing Fee

Mail your completed application along with a processing fee of $150 for one hearing aid or $300 for two hearing aids. The fee should be a cashier’s check or money order payable to Live Better Foundation (no personal checks accepted). Send everything to:

4. Wait for Our Response

After submitting your application for financial assistance, a Foundation representative will contact you to confirm receipt and provide additional steps if necessary. Please allow up to two weeks for processing.
